The CO and XO Both?

The Commanding Officer and Executive Officer of USS Tortuga (LSD-46) have been removed for "loss of confidence."

You don't usually see both the CO and the XO relieved. "Loss of Confidence" can cover a lot of territory, too. Maybe they were gay lovers?
