We learned about this in Coast Guard Auxiliary. It's sometimes called the "Station Wagon" effect - the near vertical stern causes a pocket of air to be captured and pulled behind the boat. Since the exhaust is expelled at the stern, there's a buildup of carbon monoxide.
The cure is to stop the boat and turn direction every 5 minutes or so.
