The toilet at a Carl's Jr. Restaurant in Centerville, Utah, was fatally shot when the gun of a man using the toilet slipped out of the holster and discharged.
The owner of the gun had a valid permit to carry a concealed handgun. The police confiscated the handgun, a Kahr P40, for testing.
